How do I address a coworker who consistently takes credit for team work in meetings?

Been noticing a pattern over the last couple months where a coworker presents work the whole team contributed to as primarily their own during leadership meetings, and it's starting to affect how our manager perceives who's actually driving projects. Haven't said anything directly yet since I don't want to come across as petty over something that might be unintentional, but it's happened enough times now that it feels deliberate. Looking for advice on how to address this without it turning into a bigger conflict than it needs to
