I'm 28 Male in Ontario Canada, I have extreme PE (less than 7 seconds on average).
My penis and rectum are always in pain, a sharp stinging pain, like there's barbed wire running through me.
I can't feel if I'm erect or not, I don't feel any pleasure with stimulation, I only feel pain from it being touched, I can't feel when I ejaculate besides pain in penis and testicles, and I have no orgasm.
So far I've had testicular regular ultrasound, no issues. Duplex ultrasound, cavernosal arteries are shrunk more than normal (right 20cm and left 15cm).
Haven't been diagnosed with peyronies, but I do have a curve and there was some plaque buildup.
I have been doing pelvic floor therapy, around once a month for over a year. No improvements.
I got a prescription for Cialis, it helped me be able to get erections again, but they do not last (still 7s average) and it takes forever to get hard again, even after a 20mg dose.
Where the fuck do I go next guys I'm so lost. I can't see another urologist until December (I had one who's been overseeing me since 2024 but last month he told me there's nothing more he can do). I feel so hopeless, insecure, like I'm about to get fucking castrated. If it's not gonna work I don't want to want it to work.
What else would you guys recommend
