WA state uro recommendations

Hey all, I’m dealing with my second “round” of prostatitis. Now that I’m almost 50 they also test PSA, and I’ve ended up getting an MRI which showed both moderate BPH and signs of prostatitis. No lesions, but they still want to do a biopsy for some reason…which I’m really not sure about with an already inflamed prostate.

My first prostatitis was verified pseudomonas infection, this time they’ve not found anything, but the DRE before the two glass test was -very- minimal and I’m not sure I trust it.

Prostatitis wise all I’ve done so far is one month of Docy, which didn’t do much. I have lifelong complex uro issues and feel my care providers are usually unresponsive and out of their depth.

I’m wondering if anyone would have a specific uro with a good reputation for complex cases in WA state or even nearby.

Mechanism of prostate inflammation other than bacteria

Literally now have an MRI showing prostatitis (bph too) in the peripheral zone of my prostate. -This time- (not my first battle) they have yet to find a bacteria.

I see a lot of bold claims here about non-bacterial prostatitis, and I’m curious, for those in the know, what exactly is the theoretical mechanism for an inflamed prostate in the absence of a pathogen?

In the BPH club now

Just went through the prostate MRI to screen for cancer due to PSA, no cancer or lesions (yay), but I do have a 37cc prostate and prostatitis as age 49 (50 in July). I’m having a hard time figuring out what my options are here.

I’ve been takin tamsulosin daily and it helps, but peeing is not easy a lot of the time. Is there a stickied thread or 101 thread somewhere for handling symptoms of this condition? The prostatitis is its own thing and not new to me, but combined with the BPH it’s definitely a drag on quality of life.

Focused pelvic floor pt

I have tried pelvic floor PT once, we have only one person in town. I stopped because they eventually found I had a bacterial infection, and put me on abx, which took care of the symptoms at the time.

This go round my symptoms more seem to fit CPPS, I’m currently being screened for prostate cancer, if I don’t have it I will being doing pelvic floor PT, even if I do have it, I will eventually probably be doing pelvic floor PT…but in either case it will be a while.

So, my question is, can someone recommend a video or online series for targeted pelvic PT that isn’t just some generic adductor stretches and such? A lot of the stuff I see is barely specific to pelvic floor dysfunction. Is there a video set out there that goes into a routine for kegels, reverse kegels, anal muscles, etc. that people can recommend until I am actually seeing a PT?

I am fairly versed in exercises and stretches and things like psoas and adductor stretching are not anything I need instruction on, whereas the “internal” part of pelvic PT I feel I could use some help with. Thanks in advance.

What should I do next?

Ok here’s the background, I’m 49, 50 in July. Had my first run-in with Prostatitis about four years ago, went through a bunch of sub par urologists, ended up being referred to a decent one, he thought I had CPPS (which I probably did too), then the two-glass test yielded pseudomonas infection.

Took about 7 days of Cipro until my tendons said “nope”, went up for another test, found no bacteria, symptoms slowly abated. Occasionally would have some pain but usually it’d last a couple days and I’d be back to normal.

Fast forward to this year, GP now screens for PSA because of my age, had a PSA of 4, then one of 7, and yet another that was down to 5.

So far I’ve seen the uro in town who did a two glass test but I don’t think expresses much from the prostate, test was negative, I’m on a month of Docycycline, on day 5, not sure it’s doing anything?

What would you do in my shoes to expedite care? I also want to rule out cancer reasonably and I think can push for an MRI or Ultrasound.

Symptoms are a little different this time, more perineum pain and ache, whereas my previous infection there was more urination difficulty.

Any insights or advice from vets would be appreciated.
