Lead pipe underground to toilet?

Had a service call end of day Friday which was suppose be easy, install toilet for senior citizen and possibly repair flange….this is what I found. Flange already removed and it looks to be old lead pipe underground. So right now I’m brainstorming what to do for her. Looks like the copper on the old flange was cut just enough to direct the sewage in the right direction.

I’m thinking of getting a new flange and doing the same, but maybe use pc4 , cold caulking compound to help seal the connection between flange and the old lead and concrete.
