Problem printing and maybe screen issues

Just looking for some confirmation here, but my last free prints have gone away. Random sections of my models are missing. For example, on the attached image you have the right hand missing on the model, but everything else intact. How did that happen when the supports are there?

Additionally my last two prints, when I pull off the resin tank that has the fep, under the screen but on top of the UV light display, there's resin? Is there a hole in screen? When I lift the tank up there is no resin that seems to be dripping through the fep? I clean after each print so I know for a fact no resin got under the screen/tank when I put it together. Regardless does the screen need replacing? Are these issues related?

I cannot figure out why my model doesn't stay on supports!

I have been having a hell of a time trying to get some prints of this Sqirtle figurine and I keep failing. I'm not sure why.
It's like the model is coming off the supportsg, maybe through suction or something? I'm not sure. I have no suction cups according to lychee as I put holes in the areas where it's hollow. Do I not have enough supports?
I've tried to increase exposure from 2-3 seconds, and other burn-in settings but nothing seems to work.
I am using the Mars 5 Ultra and Elegoo Ceramic Gray water washable 2.0 resin.

I live in Hawaii so it's naturally hot. I've noticed that the machine when it starts quickly gets up to 40*C sometimes gets to 45*C, but that hasn't been an issue for other prints. I read that the heat reading doesn't necessarily mean the resin is that hot, just the machine? Is this a heat issue?

There are some photos of the most recent attempt and my current resin settings. I've included a psyduck and oddish pot that printed with relatively no issues.

Any advice would be appreciated. I need this 'mon
