Help me ID what may have cause the death of my Tang

My Blue Eyed Tang who I had in a quarantine died today and I can’t figure out what may have cause the death of it since it’s been completely active and eating before this happened.

He wasn’t acting erratic or showed signs of sickness, it was eating normally. I can’t see any signs of velvet or ick but you can definitely see dark splotches on its body almost like bruises? Does anyone know what it might be?

QT Tank info:
He’s been in QT for a week and a half, the tank has a heater at 80F with an external filter, some live rock from my sump and I’ve been changing the water every 4 days from my established tank. The tanks parameters were Salinity 35, Amonia 0, Nitrite 0 and Nitrate 10.

Rock filler suggestions

Does anyone have suggestions on what I can use to fill the gaps of these rocks. At the time I thought it looked good but now I’ve grown to hate it. I was thinking epoxy but I have so many gaps I want to fill that it would cost me an arm and a leg.

Any suggestions?

Can someone help me identify these pests? And why they are not dying?

I got these montiporas as a rescue a while ago. I, as always, dipped them (seachem dip) before adding them to my aquarium. I noticed that they stopped regenerating and started to bleach again and noticed some bugs on them. I took them out and re dipped them (seachem dip) but they are not dying. They look almost like ants, can anyone tell me what they are? And how can I ensure they’re dead?!

Ignore the algae, but as I was cleaning my tank I came across this little guy. I can’t seem to identify it, can anyone tell me what it is? Should I be concerned?
