Cat not improving post vet visit & diabetes diagnosis

My male cat, whom is about 12 years old, was recently diagnosed with diabetes after a vet visit we had about a month or so ago. He’s been prescribed two units of insulin every 12 hours, which my family and I have been diligently attending to, but the problem is— he has had no improvements to his health. I know this cat like the back of my hand, and he has made seemingly zero improvement health wise, he sleeps on the ground/hides in the basement (when he was formally a snuggle bug who lived to take up space in someone else’s bed) he’s not vocal anymore like he used to be, he generally has no energy, he doesn’t groom himself anymore & has dandruff, and he continues to drink copious amounts of water and urinate constantly. His urine smells absolutely foul, and once his litter box is used more than twice, he will go to the bathroom on the floor. We have three different litter boxes for him and he still chooses to urinates inappropriately if his box isn’t scooped three times a day, we have tried to switch to wet food and other more formulated kibble to better his health and he refuses EVERYTHING else aside from his original kibble, I have (not exaggerating) tried over 20 different kinds of food and he has turned his nose up at every single one. I am really scared that he’s giving up or we are hitting the point of his life where he’s ready to go, and I just want to know if anyone has any advice at all on what to do. We have another vet visit scheduled for next week, but I’m not sure what questions to come in with or if I should put together a list of symptoms, but they were the same as last time and they didn’t have many answers for us. Please help.
