Onset & Early Progression (Feb – April)
On February 17th, I woke up with severe lower back pain without any prior injury or obvious trigger. The pain was mechanical and felt like an acute muscle strain/spasm, triggered only by specific movements and absent while lying down. Assuming it was minor, I did two kettlebell workouts. For the next two months, the pain settled into a very specific, localized spot around the L4-L5 vertebrae on the right side. During this period, it only hurt while sitting and didn't bother me in any other position. By late April, the pain began shifting toward my right glute as a dull ache, still hurting during sitting and certain movements, but staying away during walking and lying down.
Worsening Symptoms & Physical Therapy (May – June)
By late May, the issue escalated. The pain became more frequent in almost all positions and started radiating down the back of my thigh, occasionally reaching my calf and heel with mild burning or tingling sensations. Forward bending also triggered abdominal discomfort, and the pain became erratic—moving around from second to second. During 10 physical therapy sessions in early June, symptoms initially worsened, causing severe evening back pain and morning stiffness. However, on day six, after doing core exercises (planks, dead bugs, bird-dogs) and experiencing a strange "pressurized" sensation in my back, the leg pain almost completely disappeared overnight, leaving only a dull ache in my right lower back.
Current Status, Medications & Patterns
Over the last two months, I’ve been actively swimming, walking, doing pull-ups twice a week, and training my core, all of which are completely pain-free. Lying down gives almost total relief. My main issue remains prolonged sitting—after a while, my right lower back becomes deeply irritated, forcing me to shift constantly. During a 19-hour road trip, walking relieved the pain immediately, but sitting and shifting spine positions triggered a deep, diffuse ache strong enough to cause mild nausea. A short course of Mydocalm (150mg 3x/day) significantly reduced the dull pain and stiffness, whereas Arcoxia (90mg) had no noticeable effect.
Specific Mechanical Triggers & Observations
There are a few clear patterns to my pain: isometric rotation (like holding a Russian twist) instantly triggers a sensation from my lower back through the groin into my thigh, which stops the second I release. Palpating my lower back reveals a clear trigger point that refers pain down to L4-L5, and resisting hip flexion (pushing against my knee) causes lower back discomfort. Prone lying followed by back extension triggers pain in my right QL muscle. Lastly, stretching my hamstrings reproduces the pain, but the sensation disappears if I point my toes away (plantarflexion) rather than pulling them up, and bending forward quickly causes a sharp catch, whereas moving slowly prevents it.
This is a short summary of my MRI:
- Spine Alignment: Flattened lumbar lordosis (straight lower back) with mild scoliosis. Vertebrae are healthy and aligned.
- Genetically narrow spinal canal across multiple levels (9.5 mm to 10.5 mm mid-sagittal diameter), which is classified as absolute/relative stenosis.
- Discs & Nerves: Multiple dehydrated discs (polydiscopathy) with small bulges/protrusions. Because canal is narrow it make contact with nerve roots at multiple levels (L2, L3, L4, and L5), most significantly at L4-L5 on the left side.
- Schmorl’s Nodes
