If I open a PDF and change the resolution in the initial pop-up window, every new file I open afterward will reflect the resolution I added to the first file. This is keeping me from seeing the native resolution in subsequent PDF's. How do I keep this from happening? Restarting Photoshop and even rebooting doesn't fix this.

Yesterday, I got new lenses for my everyday and computer glasses. The everyday pair include Prism to correct my sight line not being perfectly aligned and a progressive bifocal. My computer glasses are single vision and have been great for looking at my monitor just two feet away. When I tried out the new lenses for my computer glasses, my vision was significantly doubled. So, I asked that they put the old ones back in. When I got home, I found that those give me double vision as well. I now have raise up one side a inch over my ear for them to be clear. I actually put a piece of Starburst candy under the right stem so I'd have some kind of reference. And yet, these lenses were just fine before having the new ones put in.

Is there some process for replacing lenses that involves altering the frames? They didn't look bent, but I guess they could be. I just know there's no way I went cockeyed by 15 degrees overnight. What factors might play into something like this?
