Hi all! I’ve spent a lot of time reading through this subreddit and was hoping to get some thoughts from people who may have experienced something similar.

I noticed through all the images that perioral dermatitis can visually appear different across multiple people, with some getting the form of pastules face wide alongside with it.

I consistently get tiny little whiteheads that pop up quickly. They mainly appear on my chin, around my nose, upper cheeks / under-eye area and around my eyebrows. Alongside this, I also get a dry, sandpaper like rash around my chin and nose.

I initially treated the little whiteheads with antifungals and switched my skincare to FA safe products. Whilst this did clear up my forehead, I am still getting reoccuring whiteheads only in the areas mentioned above.

I recently started taking the approach that I have perioral dermatisis and treating with the recommended products such as taking a zinc supplement, vitamin D and omega-3, and Avene Cicalfate Spray (FA safe) and cream (Non FA Safe) and stripping routine back to gentle cleanser and removing any SLS exposure. Sometimes I can get my skin clear for 2 or 3 days if I am having a good run, but then the little white heads pop up again.

I have tried topical metronizadole and it seems to make them worse. I am stuck between treating as bacterial vs fungal. Next steps doxy?

I know I have a mild case compared to many, but this little whitehead issue has plagued me for years so I would love to nip it in the bud once and for all otherwise my glow up 2026 year would not be complete!

Zinc is helping

I’ve shared a few updates here previously about my experience using sirolimus, but I wanted to mention something else that appears to be helping reduce my redness.

I recently developed a mild case of perioral dermatitis, which is connected to how compromised my skin barrier is. I also think this damaged barrier may be part of why I became more prone to Malassezia folliculitis and it's also known that KPRF is a contributor.

I started taking zinc supplements and stripped my routine right back to a gentle cleanser. I started using Avène Cicalfate Spray (fungal acne safe) and Avène Cicalfate Cream (not FA safe) in the evening, alternating every couple of days. They both use zinc oxide.

Since doing this, I’ve noticed a significant improvement in the baseline redness across my cheeks. I believe the approach to repairing skin barrier is going to make a big difference.

I’ve also started taking vitamin D, and I have omega-3 supplements on the way, but the zinc supplementation is the thing I’d most strongly recommend looking into based on my experience so far.
