Vent: Office Billing Reps

I went for a TCM (Transitional Care Management) appointment with the hospital affiliated PCP(2) as I was told to do if I did not have a PCP, or could not see PCP(1) within 7 days. I told them I wanted to leave PCP (1) and wanted to just make it new patient visit. She said it doesn’t work that way, they have to do TCM appt then schedule new patient.

They said there was no payment for TCM appointment, yet have a pay before appointment policy, so I thought maybe because it is part of the hospital stay, or insurance covers it to decrease readmissions.

Well the insurance denied the TCM appointment saying needs referral from PCP (1) but was coded as New Patient First Visit not Transitional Care Management.

I called billing and told them I did change to them within a day or two later so could probably call and get it backdated, and just pay $15 copay.

I said or we could do the proper thing and have it coded properly receiving insurance reimbursement by 50% or more.

The TCM reimburses more because it includes home health care coordination, prescription management, referrals, etc.

He basically ignored what I said and looked in the computer and read patient needs to call insurance and get PCP backdated. I said yes but that is based on the rejection code which in this case is different because you billed for 99203 but should be 99495 or 99496. He got real snippy and said “no, there is no error, I noted your chart that I told you what you need to do, have a nice day” and hung up. I really liked this new doctor but everything with the back office is a mess, do not actually listen to what you are saying and try to be helpful, and billing is the only department that answers the phone, everyone else leave a message and wait 1-3 days for a call and if you need some other group you wait another 1-3 days.

Has anybody had to get a catheter because of urinary obstruction caused by prolapse?

I was referred for a Kidney and bladder ultrasound and to a nephrologist following lab tests and the nephrologist sent me straight to the ER to get a catheter and I had to spend two nights in hospital.

Has anybody else had a similar experience?

Got two job offers same day.

After four months looking I had two interviews in a week and both offers at above my previous pay. I had a couple screening interviews and one 2nd interview with no offer in the first month looking and then nothing until I finally got two interviews and offers in a week. I wanted them both!

I wrote my resume to really show my experience for the job thinking of examples that were minor and showed my knowledge of the subject with tiny details.

Made up numbers on the resume just thinking okay how much time or money was saved and make your best guess. Think how much the average pay was and multiply it by wild guess at time saved to come up with a yearly savings amount or percentage and there you go. Think about how much merchandise you sold more than store average, name how and guess per day sales for maybe four outfits or accessories like sock sales and upgrades. Give it a number, unless they have a friend at the previous company, they’ll never know, companies don’t even have that kind of data unless you are Don’t be too outrageous to be unbelievable but enough to count and show your effect on results or earnings.

I didn’t provide cover letter.

I didn’t have a professional summary or about me type section at the top, which I removed after reading tips one of the companies actually had on its website that I was hoping to get hired at. They said anything in there is or should be redundant with other sections of the resume. They don’t need you to spell out how many years experience you have, they want you to show it in your experience, skills and education.

I didn’t address a two year and one year gap following layoffs that my industry is well aware of, so don’t hold it against you, maybe some would wonder why you took so much time off, but severance makes it not as much of a rush for some. It came up very briefly, they have even said oh yeah they laid off a lot of people. When the others asked, I said it was a mass layoff following whatever caused it.

I always intended to, but have never sent a follow up or thank you email after an interview, it somehow always just got past an acceptable time, I always think to myself I would actually prefer that candidates not do that when I have interviewed them, it is another email to read, would stand zero chance of changing my mind, and could always harm you if you say one wrong seemingly innocent thing. If it is too pompous you probably aren’t getting the job anyway because people that take things too seriously annoy me.

I did way better in the last two jobs I interviewed for and I think it was partly because I did some deep thinking how impressive my career has been and I was being told they chose someone with more experience when I am one of the best at what I do, so I realized I obviously wasn’t presenting that in my resume and I really needed to show receipts, and made big changes to do just that.

I think the interviews going better was largely due to the interviewers having personalities that I work well and am comfortable with. They were my people. It is the same for me as a hiring manager with personality a critical factor. If they annoy me in the interview, they’ll annoy me even more when they work for me.

Unicorn Landlord (NV)

I submitted a maintenance request recently when one of the appliances stopped functioning in the condo I have rented for a decade, and the owner bypassed the property management company and called me directly that she wanted to replace all appliances, range, microwave, dishwasher and refrigerator, which was done, and not with cheapest, she asked my approval, telling me she chose them as if it was for her own home.

The day the appliances were installed she call to ask if they were a good choice, and I said yes.

Then she said is there anything else I want to make it look good and nice to want to stay and I said well, the carpeting and linoleum is really bad, and she said oh yes, that needs to be replaced with vinyl plank and tile, do some shopping and let me know what you like, and she had the floors all replaced with the same she used in her home, which she sent pictures of to see what I thought, which I love and it is really nice and high quality.

I thought I would have to move just to get upgrades like this as other owners have all replaced the cheap materials the builder placed as tenants move out and rent is the same.
