Looking for a good gym membership form template I can customize

I'm helping a small gym move its new member registration online and I'm looking for a gym membership form template we can customize instead of building everything from scratch. We need to collect the usual stuff like basic member info, contact details, emergency contact, date of birth, membership preferences, etc. It also needs to work well on mobile since I imagine a lot of people will fill it out on their phones.

My first thought was Google Forms since it's free and everyone already knows how to use it but I'm wondering if there are better options for something that's part of the actual sign-up process. I've also looked at Jotform, which seems to have a lot of templates and features, but honestly, it might be more than we need. I also came across Youform's gym membership form template, and it looks like it could save us some time.

For anyone who runs a gym or fitness studio, or deals with memberships, what are you using for your gym membership forms? Mainly looking for something that's easy to customize, looks professional, and isn't a pain for new members to fill out.

I’m looking for an anonymous feedback tool that has a way to reassure the team that it's actually anonymous [IN]

Ideally, one with some kind of visible anonymous badge/indicator so respondents can verify for themselves.

I’ve looked at Google Forms. But it looks quite outdated. I've also come across Youform, which seems to have what I'm looking for. Before I commit to anything, though, I want to hear from people who've actually used the tool or suggest similar or better tools, if any.

People are usually skeptical when they're told something is anonymous, especially when the feedback could be critical. Ideally, I prefer something with a visible badge/ indicator so respondents can verify for themselves that their identity isn't being collected.

Which customer satisfaction survey would you recommend today?

So, I need to send a 10-ish question survey. I've looked at some tools but all seem more apt for huge teams, which require me to get on a call before I can even figure out their pricing. I'm thinking between Youform and SurveyMonkey. Youform seems to cover what I need without paying, while SurveyMonkey's free plan is limited to 10 questions, meaning I'd need a paid plan for what I need done. Anyone used these specifically for surveys? I just want to send out the link asap and start getting answers.
