not much changes on this timeline, here just happends that Manuel Dorrego is more firm on the recently captured misiones orientales and UK accepts this for a stronger buffer state.
The guerra grande didn't change much, with the northern territories of uruguay acting as a decent hiding spot for early rigrandense revolutionares and as a strong base for the general Fructuoso Rivera, the one to capture the provinces in the first place.
i think uruguay is in a slightly better position, for modern times, thanks to this littel bit of more land and resources, and leadding to better internal borders for the nation.
also in this timeline uruguay participates in the 1934 world cup and wins, having five stars in the modern day, thanks to this small chunk of territory.

so basically in this timeline the inca plan was a lot more suported in argentina, but buenos aires still cancelled it, then, during the civil war between federalist and centralist when the federal league forces captured buenos aires the congress of tucuman decided to go forward on the inca plan electing Juan Bautista Túpac Amaru as king; another change is that artigas forces managed to start an earlier riograndense revolt, peacing out with the portugese, once Francisco Ramírez and his allies decided to betray artigas , transofrming the war into one between: artigas' uruguay, the rest of the federal league and the second inca empire.
After months of war, where the federal league were limited to just buenos aire, the UK entered to broker a peace deal, where buenos aires was given independance as a neutral state and artigas managed to regain the titel of federal leageu as well as all the territories up to cordoba.
At the same ime all of this was happening Jose de san martin was liberating chile and souther peru for the inca empire (as he was a suporter of the monarchy) with an enfacis for cuzco to make it the new capital. Simon Bolivar was also liberating northern peru, when they both met in discussion, bolivar agreed to help give souther peru and bolivia to the inca kingdom (also bolivia vote to join the inca kingdom) as northern peru became an independant state.
The new inca kingdom still was plagued with instability as any good latin american nation, wich led to civil war erupting in the nation, at the same time buenos aires had a civil war, where part of the inca state joined to support one team, leadding to the federal league joining for the other and to formalise their border, at the same time paraguay joined the fight against the inca for the chaco region and other territories; at this time, the young brasilian empire also joined the war to crush and regain rio grande as well as other territories, fighting alongside the inca, in an alternate version of the " Guerra Grande" in wich the borders of all nations where formalised, with paraguay gaining the chaco region and other territories, buenos aires was formalised as a neutral state, rio grande was formalised as a new state and the inca civilwar was resolved.
After that war brasil continued weaker than in our timeline, loosing some border territories to incan settlers.
The inca empire continued trying to expand into patagonia, but being too overstreched and british and french intervention a mapuche kingdom end formalising in patagonia.
The federal league of artigas, who was very friendly to britan, was allowed to keep the malvinas islands from spain, as the federal league, paraguay and riogrande grew to become some of the richest nations in south america.
The second inca empire, even though big and powerfull, still continued being plagued by corruption and instability (like peru, bolivia and argentina of our timeline).
Gran colombia still colapsed the same even though they had an incan ally.
After the napoleonic wars for not any special reason the dutch were given french guyana; and british guyana didn't expand as much.
