Hi everyone,

I'm confused about the Employer Contact Details section in the VETASSESS application.

My Statement of Service was signed by HR, and my former line manager is no longer with the company.

HR team provided the document with Director HR's signature, doc verification email, and no phone number. The person working on the documents wasn't the Director, it was someone junior.

For the contact details, should I enter:

1. my former manager (no longer at the company, email would be with some other company domain)
2. the HR Director who signed the letter (his work email & phone number)
3. generic verification contact details? (doc verification email and company phone number)

I'm working on creating the Organizational chart which I'll get printed and signed on company letterhead. Can someone please share some samples? Online I only see a photo of a chart, but I want to know how the official document looks?

Help!
