I had a biopsy on two thyroid nodules performed today

I swear it was one of the worst things I’ve ever experienced in my 28 years of living.
Thank God my husband was there to hold my hand bc the pressure and pain deep inside my throat was super intense and uncomfortable.
Plus, my husband said on all times (8 times) the only thing the came from the needle was a clear fluid. No blood. Is it a good sign?
One nodule was 1,8cm trad5 and the other 1,4cm trad4

Tenho 28 anos e descobri ontem que tenho múltiplos nódulos na tireoide, no lobo esquerdo. Sinceramente, estou apavorada porque um deles é altamente suspeito, e minha consulta de acompanhamento com o médico é amanhã. No entanto, meus hormônios da tireoide estão perfeitos, apesar do meu histórico familiar de hipotireoidismo.
Aqui estão os achados exatos do ultrassom:
Lobo Direito e Istmo: Tamanho e aparência completamente normais (ecogenicidade homogênea, fluxo de cor normal).
Nódulo 1 (Inferior Esquerdo): 1,8 x 1,4 x 1,7 cm. Sólido, hipoecoico, margens lobuladas/irregulares, com macrocalcificações e focos ecogênicos puntiformes. Categoria ACR TI-RADS: TR5 (10 pontos).
Nódulo 2 (Médio Esquerdo): 1,6 x 1,4 x 1,4 cm. Sólido, hipoecoico, margens lisas, sem focos ecogênicos. Categoria ACR TI-RADS: TR4 (4 pontos).
Meu Histórico Médico e Contexto:
Tenho SOP (Síndrome dos Ovários Policísticos) e fui diagnosticada no início deste ano com resistência grave à insulina. Minha insulina de jejum estava em 37 µUI/mL (o normal, idealmente, deveria ser abaixo de 10).
Estou tomando 1000 mg de metformina há cerca de um mês e, felizmente, estou tolerando muito bem.
Pista Temporal: Há cerca de um ano e meio ou dois anos, eu sentia pontadas agudas, repentinas e aleatórias, bem na região da tireoide. Elas pararam, e não apresento nenhum sintoma sistêmico ou compressivo no momento.
Meus Medos e Ansiedade:
Ver "TR5" e "Categoria de Risco Máximo" fez minha ansiedade disparar. Tenho pavor da palavra câncer, embora saiba que problemas na tireoide são altamente tratáveis. Fico me perguntando como posso ter nódulos calcificados ou cicatrizados "antigos" com apenas 28 anos. Estou tentando desesperadamente descobrir se existem casos com características como as minhas que acabaram sendo completamente benignos. A Hipótese da Relação com a Insulina:
Tenho lido sobre como níveis elevados de insulina em jejum (como o meu, de 37) atuam como um potente fator de crescimento para as células da tireoide, forçando-as a se multiplicar excessivamente e a formar aglomerados densos, sólidos e escuros (hipoecoicos). Suspeito que aquelas sensações de pontadas aleatórias que senti há dois anos tenham sido surtos de crescimento benigno que, com o tempo, deixaram para trás tecido cicatricial microscópico (fibrose) e calcificações — características que o ultrassom agora aponta como altamente suspeitas (classificação TR5), embora possam ser apenas "resíduos" de um processo estimulado pela insulina.

Alguma de vocês com SOP e níveis elevados de insulina já teve um nódulo classificado como TR5 que acabou se revelando totalmente benigno?
Os nódulos diminuíram ou se estabilizaram depois que vocês reduziram a insulina em jejum com Metformina ou mudanças no estilo de vida?
O que devo esperar do procedimento de biópsia por punção aspirativa (PAAF)?
Vou consultar meu médico amanhã para discutir a biópsia, mas meu coração está acelerado hoje à noite. Qualquer palavra tranquilizadora, relato de experiência ou história de sucesso seria de enorme ajuda para mim agora. Obrigada!

Right Lobe & Isthmus: Completely normal in size and appearance (homogeneous echogenicity, normal color flow).
Nodule 1 (Left Lower): 1.8 x 1.4 x 1.7 cm. Solid, hypoechoic, lobulated/irregular margins, with macrocalcifications and punctate echogenic foci. ACR TI-RADS Category: TR5 (10 points).
Nodule 2 (Left Mid): 1.6 x 1.4 x 1.4 cm. Solid, hypoechoic, smooth margins, no echogenic foci. ACR TI-RADS Category: TR4 (4 points).
My Medical History & Context:
I have PCOS and was diagnosed earlier this year with severe Insumin Resistance. My fasting insulin was clocked at 37 µUI/mL (normal should ideally be under 10).
I have been taking 1000mg of Metformin for about a month now, and thankfully I am tolerating it very well.
Timeline Clue: About 1.5 to 2 years ago, I used to feel sudden, random, sharp poking sensations, right in my thyroid area. They stopped, and I don't have any systemic or compressive symptoms right now.
My Fears & Anxiety:
Seeing "TR5" and "Maximum Risk Category" has driven my anxiety through the roof. I am terrified of the word cancer, even though I know thyroid issues are highly treatable. I keep wondering how I could have "ancient" calcified or scarred nodules at only 28 years old. I am desperately trying to find out if there are cases with features like mine that turned out to be completely benign.
The Insulin Connection Hypothesis:
I’ve been reading up on how high fasting insulin (like my 37) acts as a massive growth factor for thyroid cells, forcing them to over-multiply into dense, solid, dark (hypoechoic) clumps. I suspect that the random poking sensations I felt 2 years ago were benign growth spurts that eventually left behind microscopic scar tissue (fibrosis) and calcifications—which the ultrasound is now flagging as highly suspicious TR5 features, even though it might just be the "dust" left behind by an insulin-driven process.

Have any of you with PCOS and high insulin had a TR5 nodule turn out to be completely benign?
Did your nodules shrink or stabilize once you got your fasting insulin down with Metformin or lifestyle changes?
What should I expect from the FNA biopsy process?
