Engine Running Too Lean [Code P2096]

Picked up this 2010 MazdaSpeed3 at 108K miles a couple of months ago. Couple of weeks ago it started throwing a P2096 code w/check engine light, where only the downstream 02 sensor says it's running too lean. Took it into my mechanic and he saw that the downstream sensor was bad, and so he replaced it. Check engine light came back on shortly thereafter; I cleared the code and the problem cropped up again.

I'm thinking it might be an exhaust leak, but would like to know if anyone has dealt with similar problems; this is my first Mazdaspeed3 (first manual vehicle I've ever driven, actually).

My First Ever Tank

Setting up my first ever aquarium as a nano reef tank. Need some assistance with keeping my macro algae healthy and water chemistry balanced.

I’ve been adding a pinch of nano fish food to it daily and have also added 2 bottles of bacteria to it for my nitrogen cycle since setting it up.

Some of the Red Ogo seems to be rather pale and struggling a bit; I assume this is due to the Sea Lettuce leaching nutrients from the water. I will note that the Sea Lettuce grows FAST; I’ve had to harvest it a few times.

I also have these white, tendriled specks showing up on the glass. I’ve checked the ID thread in the rules and I didn't see anything that looks quite like them on there, and attempting photo ID with AI has proven unhelpful.
