Hello all.

I have a 75yo patient that wants some opinions on fixing her current mouth.

I did an exam and am kind of at a loss. Her budget isnt really finalized but I got the sense that 15,000 USD was about the max.

Things Ive noted were the large palatal torus and moderate perio in the upper posteriors

I considered an UCP on the upper but I'm not sure how stable that would be. Does anyone have experience with a Kennedy class IV partial that replaces 8 anterior teeth? It would avoid the tori issue but patient has to accept risk of failure if she looses another tooth.

The other option is to extract the uppers and go to an upper denture or Overdenture but I'm concerned about the tori and it doesn't seem like she's up for surgery to remove it. So maybe a horse-shoe Overdenture?

The lower teeth are in good condition other than some enamel wear. I think a lower cast partial with or without terminal implant support would work.

What are your thoughts? I'd love to hear it.

Injection Molding for Lingual wear

Hello all. This was an interesting case for me. My first case of injection molding.

Patient was missing posterior teeth on the Maxilla and had severe lingual wear on the remaining teeth.

I ordered a lingual waxup at the same vdo so she could keep her existing upper partial and had the lab grind off 1-2mm of the lower incisors and then waxup the uppers. The goal was to get evenly distributed contacts in MIP and smooth excursion movements.

There is an air bubble on the lateral that was easily repaired. I kinda messed up grinding the lowers as I think I went too aggressive and lost some contacts.

Anyways, it was a learning experience for me as it was my first injection case and I gave the patient a hefty discount for her to be my guinea pig. The only issue is she states it's harder to tear her food now... Dunno how to fix that problem 😮‍💨

I know I'll probably get some hate and this radiographically doesn't look as good as I want it to but this patient was a PITA to work on and I feel like I did a good job despite everything.

Caries dye indicator used so I'm not certain the radiolucency under the buildup is decay.

Incisal composites = Cheat code

Not sure how many other docs here are taking advantage of this but dentin wears at about 8x the rate of enamel.

If a patient has incisal wear that's exposing the dentin, it's a great service to remove about 0.5mm of dentin and then cover it back up with resin.

You can knock out 6 teeth in 20 minutes and it's great production.

Am I crazy or is this crazy?

Bought these Titan handles for T-Bar Rows off my pritchett pad and instantly felt something was wrong with them.

Looking at them it's quite obvious. When you grip the handles, because of their angle, the metal framework provides NO resistance against your grip. The only resistance preventing the bar from falling back down is the friction of your hands against the rubber grips! Rubber grips which, by the way, can pretty easily slide off the metal framework!

Absolutely makes no sense. Am I crazy? I've tried playing around with them but can't see anything I'm doing wrong. How can something so obviously faulty make its way to mass production?

FA Hyper Belt Squat Attachment

Hello. I'm saving up for the FA hyper pro but, in the meantime, I've also been thinking about getting setup with some type of belt squat function. However, if I continue to save up, the FA belt attachment is only $100 on top of the hyper pro.

Does anyone have the belt attachment? How good is it? I know you can't go crazy heavy on it but I'm not a heavy lifter (I barbell squat 225).

First composite veneer case

Used a total of 5 different resins (semi-achromatic, opaque, body, flowable and opalescent) but likely could have used two (body and opaque) and had a similar result.

Midline is canted and developmental grooves need to be more subtle but patient was happy.

Case was broken into two appointments due to operator fatigue 😇.
