PCOS or CAH?

So I'm still going through the motions of managing my symptoms and seeing my provider for everything I've experienced over these last few months.

She had me repeat labs to see if there's consistency with my DHEA S being elevated and of course it still is (it actually went up) So now she wants me to do a CT scan because she now believes this is an adrenal problem not an ovarian problem. I've been reading up on the possibilities she told me which is either a growth/tumor on the adrenal gland, or a condition known as Congenital Adrenal Hyperplasia (CAH)

Basically I found out that CAH can mimic PCOS. Both have similar symptoms, but of course they're different in the way they're treated and managed. I also got a referral for an endocrinologist so I'll be seeing and speaking to them soon as well. I'm just ready to finally get answers since I've been doing this song and dance since 2020

Just got my labs done again, but this time it's actually different

So about 2 months ago I started recently seeing a new Healthcare provider and this time I actually have hope in seeing the bigger picture. For years it's always been "Everything in your labs came back normal" "Nothing to flag or be concerned about" "Just get on birth control" etc yadayadayada

But my new provider wanted me to get labs done again and she let me know that this was an extended panel, not like ones I've done in the past. So I fasted and went to have my blood drawn, a few weeks later they came back. The few things she did flag are as follows:

-Vitamin D deficiency

Vitamin D was very low, so I was prescribed a Vitamin D supplement once a week for 12 weeks. Then can switch to an over the counter

-DHEA Sulfate and PCOS Evaluation

This one was interesting to me since I've never heard of this until now, but she said the DHEA Sulfate was significantly elevated which can be in correlation to PCOS or less commonly, a growth on the adrenal gland. But she wants that repeated in 6 weeks and an ultrasound to take a look at the ovaries

-Insulin Resistance

Even though my glucose and A1C and all that other stuff came back normal, and even though I'm not prediabetic or diabetic, I'm still showing a mild case of Insulin Resistance. Options to treat include of course diet and exercise, as well as medications such as metformin, etc

-Iron

My ferritin was still technically within normal range, however its on the low end of normal so if I'm not careful, it could continue to drop and risk becoming anemic. So I was recommended a multivitamin that contains iron to help with that

So with all of that now I have more to think about. Should I start metformin? Or possibly something else like wegovy? It definitely gave me more answers, just trying to figure out how to go about it all
