Doubling auto discounts

We are a high volume restaurant that offers happy hour. We used to have a dedicated menu for happy hour food and happy hour beverages but we recently (~5ish months) changed to our default menus and add an automatic discount that is applied to eligible items, within the happy hour times/ days. This has cut down on having an extra 4 full menus, including a second menu of 40 draft beers that have rotations.

The problem we are facing is we do have instances where the happy hour discount is applied twice. For our wine, happy hour is half priced bottles, so a 50% discount is applied when the item is sent. However, randomly, when the guest is ready to check out, every single item that had a happy hour discount applied to it will have a second or third discount applied, resulting in a free bottle of wine or even a negative balance due, in the instance of wine.

Toast has not given me an answer as they have not been able to recreate the issue, even when we show proof on our side, and even though it happens at least 2-3 times every shift. Does anyone know what causes this to happen or a way to prevent it without just taking out the entire happy hour system?
