I've just started a fan account on twitter. What are the precautions I should take in order to avoid being suspended?

Does the profile picture of the person I'm doing a fan account of matter?
Do I need to add something to my "name" or in my bio?
Will the header also be a factor?
how does premium subscription work and will it cause my account to be suspended even with the fan account label?
Posting fansite pics and updates off other platforms allowed?
How does crediting work?

I'm a newbie on Twitter since I've moved from instagram since my fanaccs have been chain banned for integrity. I would really appreciate the help and info.
