How to Submit At-Risk Designation Form - Utah Voter Privacy PSA - CUTOFF DATE 5/25
If you are a victim of abuse or are at risk of domestic abuse OR live with someone who is at risk of domestic abuse, you can request to be exempt from the upcoming public release of your personal voting information. The cutoff date to submit this is this coming Monday.
You can go to vote.utah.gov - additional voter resources vote privacy - download form at the bottom.
Or you can simply use this direct link to the form: https://vote.utah.gov/wp-content/uploads/2023/06/At-risk-Designation-Request-Form.pdf
Fill it out Mark domestic abuse as the reason - you do not need to provide any details in the box at the bottom.
You can email it. You do not have to drop it off in person.
