5 months post-op: ACL revision + Agili-C still dealing with medial condyle pain in early flexion

Hey everyone, wanted to share my situation and see if anyone has had a similar experience.
My procedure (January 23, 2026):
I had a pretty complex right knee surgery that included:
• ACL reconstruction (3rd time) using a quad tendon graft
• Lateral extra-articular tenodesis (LET)
• Partial meniscectomy
• 3 Agili-C osteochondral scaffold plugs — 2 on the medial femoral condyle, 1 on the lateral
The Agili-C portion was the part I knew least about going in. My surgeon chose it to address the chondral damage that’s been building up over years of instability and prior surgeries.
Where I’m at now (~5 months post-op):
ROM is sitting around 115–125°, quad strength is still lagging (LSI around 37%), and I’ve been doing PT twice a week. Progress has been steady but slow — which I understand is expected with Agili-C given the longer biological timeline for scaffold integration.
The pain I’m dealing with:
The main thing bothering me is a pretty consistent pain right at the medial femoral condyle during roughly the first 10–20° of flexion under load. It’s most noticeable on the leg press and going up/down stairs. It’s not sharp, but it’s there and it nags.
I’m trying to be patient but I’m curious whether others who’ve had Agili-C plugs on the medial condyle experienced this kind of localized pain at this stage, and whether it eventually resolved as the cartilage filled in.
Anyone else 4–6 months out with a similar picture? Would love to hear what your recovery looked like.
