IM vs GIM billing

I recently found out that US internists cannot bill the same as Canadian internists. Some of the codes are restricted for US internists. I was looking at the codes and it seems like the difference in pay can be a lot for the same kind of work/counseling. I wish I had known this a little earlier. With 20-30% overhead fee, it looks like we are at significant disadvantage. Only way to access GIM codes is to get FRCPC recognition by doing 1 extra year of fellowship. I would appreciate it if any US internists who are currently practicing in BC can explain it. I am about to move next month. I wish I had found this earlier.

Pediatrician - outpatient only in BC?

Hi all,

Any pediatricians here that do outpatient only in BC? Seems like we have a lot of complex psych burden to manage so have had to make my teenage visits for psych 1 hr. With this kind of trend, I am not making much. Currently 4.5 days patient facing hours, but spending loads of unpaid hours on notes, inbox etc.

Any other outpatient peds struggling with finances? Also paying back 6 figure loans, overhead is 25%. The EMR in canada (OSCAR pro) are revolting with click fatigue, don't have in-built growth charts that calculate exact percentiles (like Cerner did). Lots of windows that pop out instead of staying in the EMR window. Dot phrases cannot automatically add values from chart into your note. etc etc.

Please reach out, would love to hear strategies on how you've been able to make workflow efficient and maximize earnings. Thanks!

How difficult is it to practice Internal Medicine or Cardiology in Canada after completing residency in the US?

Hi everyone,

I’m an MBBS graduate from India and will soon be starting Internal Medicine residency in the US. My long-term plan is to pursue a Cardiology fellowship after residency.

I’m wondering about the possibility of eventually moving to Canada and practicing there.

For someone who completes US Internal Medicine residency + a US Cardiology fellowship, how difficult is it to obtain medical licensure and start practicing in Canada?

Specifically:

  • Is US residency/fellowship generally recognized for Canadian licensure?
  • Would I need to repeat residency or fellowship in Canada?
  • Are there significant differences between the pathways in provinces such as Ontario, British Columbia, Alberta, etc.?
  • How difficult is it for a US-trained IMG to obtain a provincial license and find a cardiology position?
  • Are there any additional exams, assessments, or training requirements?

I’d really appreciate hearing from anyone who has gone through this pathway or has experience with US-trained physicians moving to Canada.
