Need advice and also i want to vent a little

I have always been an above average student when i was little, then covid came and i think my breaking point was when i had covid and even though i was weak i tried to do everything to be at the top, and then burnout and depression followed. During those days i had picked up phone addiction and sleeping. I dont have those depressive episodes anymore but the addictions still linger, i am glued to my phone, and when i am not touching it, when i am bored i just sleep. I haven't studied properly in the last 4 years, I barely pass exams. I have an upcoming exam and i want to improve, I want to become better and be like i used to before but it's really hard since the habit is quite strong. I feel ashamed of myself actually, i dont even want to talk about it with people in real life. If you have any experience or advice I would be glad to hear it.
