I want to have a epic confrontation with Setthra, not start right next to and end him instantly, same for the vice-versa, I can not imagine being Setthra and being able to play the game as slow as tomb kings need to play with Nagash right next to me.

Only reason I could guess he wouldnt start in Nagashizzar, is because he would be in the way of Kalhida doing her own thing going after Neferata, that is unfortunate for sure, but unless something is done to create at least some space in the map for everyone she wouldnt really be able to go after neferata early on, since her campaign mostly ends up going south and west, due to either skaven dwarves or lizardmen deciding you are a big problem for them
