How do I write a PS for accommodations?

Hi everyone! I'm planning on taking the MCAT in March or April 2027, and after doing some research I saw that if you want accommodations I should start the process now. I have a diagnosis from a psychologist, written documentation, and history of accommodations throughout undergrad starting sophomore year. The only thing left really is drafting a personal statement. I need help with making a good one because accommodations are very important to me. I struggle with test taking, and accommodations literally could get me a 515+ versus a 505. My GPA shot up significantly after getting them in undergrad and it helped my confidence a lot. Without them I probably would have had to take a post-bacc after graduation, but thankfully I worked hard and now I graduated with a solid GPA.

I am not super worried given I am very prepared but still, it's not 100% and I may have to appeal. Does anyone here have any experience with writing an accommodations personal statement? What to include? How to make it convincing?

"Hospitals are understaffed" my fucking asshole bro. I have been applying for months and nothing. I don't have any certifications so that might be why but I still have clinical exposure so I'm not starting from scratch. So many applications and still not selected, not even for ones that provide training. I'm not some stupid kid I'm about to have a fucking college degree I've worked in medical offices before why am I not trainable? I thought my only option was to get a CNA license but I don't have money for even that so idk what to do. Any suggestions?
