POST MMA Recovery Timelines?

I'm almost 5 months post-op 17mm advancement. My improvements are noticeable but MINOR... not enough to live a normal life yet. Still classic stuff: fragmented sleep, thick brain fog, sleep deprived fatigue....

PSG at 3 months: RDI dropped from 32 to 16, AHI dropped from 15-20 range down to 5 so far (PSG done at 3months post op.)

My spontaneous arousals are still dozens per hour: 35/hour. <-- My guess is there's some residual respiratory events in there? maybe some neurological misfires form years fight/flight resp. events?

I've heard some people feel noticeably better at 3 months, 5/6 months, 7 months, 9 months and even beyond 12 months?

THIS FEELS CRAZY SLOW... IF AT ALL. Timeline patterns anyone post MMA????? Looking for hope lol.

Confusing data

I'm using an Airsense 10. AHI 0.3 according to the machine last night, pretty typical result. I thought I had this dialed in.

But I got a recording pulse ox from Amazon, "EMAY", that gave me this data last night. It looks like my SPO2 is consistently low and my heart rate high throughout the night. Am I reading this right? Are these devices pretty accurate?

Help! OSCAR Data tuning for VAuto 11

Hi everyone, looking for some insight on my OSCAR data. I'm a 19M dealing with UARS/mild OSA, and I keep waking up frequently on my BiPAP. I got this BiPAP and am manually titrating it, so it's been a bit of a challenge. I've only recently gotten a turbinate reduction, so I'm still used to mouth breathing while unconscious, although when I'm conscious I don't anymore.

My Setup & Specs:

Machine: AirCurve 11 VAuto (VPAPauto mode: Min EPAP 7.0, Max IPAP 13.0, PS 1.6)

Gear: Nasal pillows mask P10, Micropore mouth tape.

Anatomy: Mild to moderate recessed jaw. Habitual mouth breathing when sleeping sometimes due to resolved chronic nasal blockage. Lip incompetence also due to the mouth breathing habit. 187 cm (6'2 ish), around 185 lbs (or 84kg).

My non-REM sleep is fine I think, although the flow rate diagrams could be too sharp, but during REM, I think my muscle tone drops and my tongue falls off my palate. I believe this causes the VAuto to make the EPAP up and that wakes me up. I'll also note that when I wake up my cheeks are puffed out. Maybe my mouth tape isn't holding?

OSCAR Images Attached:

Image 1 (Non-REM Flow Rate): I think the waves are decent but could be too sharp and sometimes are not good.

Images 2 and 3 (REM Zoom): Super variable and definitely does not look good.

Images 4, 5, 6: Zoomed out OSCAR data. I noticed that my tidal volume is low especially for someone of my size.

My Questions:

How do I get the REM wakeups to stop? It's really annoying and making it worse than just not using the machine.

Switch to Fixed S Mode? Would locking the machine flat at a higher EPAP and PS (e.g., 8 EPAP / 2.4 PS) be better to stop it from chasing flow limits and dumping pressure on me during REM?

How does my OSCAR data look? I'm specifically curious about the flow rate in non REM and REM, but if anything looks abnormal please mention and I'll send a zoomed in screenshot.

Increase Rise Time? Would slowing down the rise time to 300ms or 400ms soften the inhalation wave enough to preserve my tongue seal, or will that cause flow starvation?

Is a Hybrid Mask Worth It? I want to avoid a full-face/hybrid mask because the straps pull back on my recessed jaw and I don't want to reinforce my old mouth-breathing habit. But if I can't hold a nasal seal in REM, do I have another option?

Can someone help me understand my result?

So I did sleep apnea test at home. I actually slept 5 hours(according to my watch and I woke up 5 times, 3 to pee). As I understand I don't have obstructive sleep apnea. But there are 2250 Flow Limitations. I did the test mainly because I am waking up frequently and most of the times it takes me a while to go back to sleep. Ruled out diabetes, did a lot of blood tests, everything is cleared.

Inspire causing genioglossus hypertrophy?

I (43M) had inspire implanted ~20 months ago, and had Inspire on for 8 months (1.2-1.8v). It didn't work well for me, even combined with CPAP, so I didn't activate it in the most recent ~12 months. I don't have issue with the nerve stimuli or it's often inaccurate breath prediction. Just the tongue movement was not large enough for me, expecially in later half of the sleep.

Recently I noticed one side (the side of hyperglossal nerve stimulation, as Inspire is one-sided stimulation only) of tongue base is clearly larger than the other side.

I suspect repeated stimulation of the tongue muscles promoted muscle growth, at least in my case. Any other adult male have similar issues with long term Inspire usage?

SDB so common because ?

We are flat-faced apes, with more bone development resources allocated to house a larger brain, and less resources (such as stem cells for bone formation) allocated for jaws -this is definitely not the whole story of why modern human skull formed this way.

As SDB is very common in those flat-faced dog breeds:

Nocturnal Hypoxemia but no apnea

I recently got my sleep study results back. I do not technically have apnea (hypopnea index was 3.2, so not high enough for apnea, but my SpO2 dropped as low as 78% and I had about a half hour with 88% or lower).

My provider prescribed oxygen for overnight usage. I guess I'm just wondering what to expect. Any tips for adjusting? Will it help me feel better during the day?

Boyfriend [31m] gets up 3-6 times a night

This is NOT asking for medical advice. Just advice for sleep. He’s always had a small bladder but at night it’s especially bad and these past few weeks is some of the worst it’s ever been. He gets up to pee anywhere from 3-6 times a night and still gets up for work at 6:45 and drives an hour to/from work and is exhausted and miserable because he never hits REM at night because he’s peeing every other hour. He takes all sorts of vitamins or sleep assistance gummies to help him sleep but they don’t really work. His retired pharmacist parents try to give him stuff, not sure what, but that doesn’t help either. Sometimes he’ll stop drinking water hours before bed and that doesn’t help.

He and I know both he needs to go to the doctor. He just started a job about a month and a half ago and the boss is pretty terrible and fires people left and right. For context, This week was someone’s birthday who’s been there 4 years, after his assistant reminded him about the birthday, he said he doesn’t celebrate birthdays, especially at work. And that he wouldn’t even celebrate his son’s birthday if his wife wouldn’t force him. It’s a small blue collar workplace. He works in a sales position as the only salesman and is expected to make 100 calls a day (I don’t think that’s even possible. Boss said he just threw a number out there and stuck with it) and is being tracked on company software to make sure he stays busy. We all know he needs a better job, but after applying to dozens, his friend got him a job at this place and she herself is constantly overworked and stressed out.

We’re about to move in a few weeks and we can’t afford for him to quit right now. So even when he’s working remotely, I don’t know how he’ll manage to find time to do an interview nevertheless go to a doctors appointment when there’s such a risk of getting fired which we really can’t afford right now. It affects both of us as I wake up every time he gets up and I work a full time job as well. He last got up around 5:15 and is up again as I type this at 6:35am. Please help me think of what to do
