Hello, so I plan to apply sa philhealth through online as SELF-EMPLOYED. However, for now, minimum lang yung gusto kong bayaran. Sa pag fillout ng pmrf sa may Member type, plan ko na ilagay 10k sa income, pero wala ako malagay sa proof of income. Okay lang ba na i leave blank ko nalang yung proof of income?
(hindi pa ako maka punta sa physical office kasi wala talaga time, so online nalang muna ako)
Hello. Last January 2026, I registered in BIR (Form 1901) for the first time. Yung contract ko is from January 2026 to July 2026, so my total income for that time does not reach 250k pesos. So, hindi ako binigyan ng COR.
After my contract this July, ma eextend siya ulit until December 2026, and by that time, mag eexceed na sa 250k yung income ko. Ano bang form ang need ko i fillout or do I need to update some info para ma obtain ko na ang COR ko?
Hello nung nag register ako first time sa SSS, chineck ko yung sa wwbsite at nakalagay na OFW ang membership status ko. Eh as far as i can remember never ko naman pinili ang ofw huhuhu..
Also, pag check ko sa mobile app, everytime mag generate ako ng PRN, "0" at "Invalid membership type" Lang talaga ang pagpipilian.

or from "OFW" to "Self-employed" sa SSS Form E4?
Note: Isasabay ko na rin ang pag change ng membership status ko from "temporary" to "permanent"
Hi. I have applied for SSS number online last December. Upon completion and receiving my number, nag try ako log in sa My.SSS portal. Tapos, yung nakalagay na membership type sa web is OFW. Hindi ko naman pinili ang OFW. Self-employed po kasi ako.

Hello, nag apply ako ng SSS number online last December. Successful naman ang application at na receive ko na ang number ko. However, need kong pumunta ng branch and mag submit ng Form E4 with required document since tagged ang application ko as ""APPLICATION THRU THE WEB - NO DOCUMENTARY REQUIREMENT SUBMITTED"
Tapos, when I tried to check sa My.SSS app, specifically yung membership type, ito yung nakalagay. Yung "0" and "Invalid Member Type"
Also, when I visited the My.SSS portal, yung membership type ko naman is "OFW" which is weird kasi wala naman akong pinili na OFW during my application. (photo below for reference)
Ganito ba talaga ang lumalabas if prior registrant ka and di mo pa napa change ang status mo from temporary to permanent? (hindi pa kasi ako nakapunta sa branch since December after my application kasi super duper busy)
