5mg worked for 3 months, but now weaker suppression + worse diarrhoea?

Please help. 5mg originally gave me excellent appetite suppression and removed all cravings for 3 months. I then dropped to 2.5mg because of diarrhoea, but lost all the benefits so then went back to 5mg two weeks ago.

This time the appetite suppression is weaker on 5mg (food nose and cravings still present), and the diarrhoea worse, lasting longer.

I desperately want to increase to 7.5mg to achieve the suppression I got previously, but the diarrhoea is debilitating.

Im worried the prescribers will stop prescribing glp-1 meds if i come clean about it as i have had kidney injury in the past (they’re strict in the UK)? I am desperate to lose weight.

What should i do? Should i just wait a few more weeks to see if the diahrroea reduces? Or just jump up to 7.5mg? I want the cravings and food noise gone again.

**TL;DR:**
5mg initially gave me strong appetite suppression, but after dropping to 2.5mg and returning to 5mg, the suppression is now much weaker while the GI side effects — especially diarrhoea — are worse and lasting longer. Unsure whether to increase the dose or focus on getting the side effects under control.
I’ve been on Mounjaro for around 4 months and I’m trying to balance appetite control against worsening side effects.

**First \~3 months (up to 5mg)**

**Suppression:**

\- Very strong
\- Significant weight loss
\- No bingeing
\- Food noise and cravings disappeared
\- Even lost my usual desire for chocolate

**Side effects:**
\- Diarrhoea for around 3 days after dosing
\- Sulphur burps
\- Unpleasant, but manageable

**Dropped to 2.5mg**

**Suppression:**
\- Much weaker
\- Food noise and cravings came back strongly
\- Binge urges started reappearing

**Side effects:**
\- Much improved
\- Less diarrhoea
\- Overall much more comfortable

**Back on 5mg for the last 2 weeks**

**Suppression:**
\-Definitely better than 2.5mg, but **much weaker than it was previously on 5mg**
\- Food noise and cravings are still there, including chocolate, fries etc.

**Side effects:**
\- Diarrhoea is worse than before
\- Lasting longer
\- Mainly happening overnight
\- More disruptive overall

**Overall**
I’m now getting **less appetite/food-noise suppression than I originally got from 5mg, while having worse and longer-lasting GI side effects**.

I already take psyllium husk and eat a high-protein, generally low-fat diet, although since the cravings returned I’ve been eating more chocolate/fries than before.

Part of me wants to increase the dose to try to get the original suppression back, but I’m concerned that it could just make the diarrhoea worse.

Has anyone had this pattern?

**Did the suppression return after staying on the same dose longer? Did moving up help? And what actually helped your diarrhoea?**

Not Sure What The Best Next Move Is From PV Governance Experience

Honestly finding the market pretty rough at the moment for governance focused PV roles. Most openings seem geared toward case processing or senior specialist roles, which has been pretty frustrating. Recruitment also feels extremely slow right now, especially for governance/documentation focused backgrounds.

I’m UK based and have several years of experience in PV governance/documentation, mainly around PSMF updates, QPPV support, inspection readiness, vendor oversight reporting, and Veeva workflows. My experience sits much more on the governance/documentation side rather than case processing, which feels like a difficult middle ground in the current market. Starting to feel a bit stuck on what the most realistic next move actually is from this kind of background.

Trying to work out whether adjacent areas like QA/QMS, regulatory ops, TMF/eTMF, inspection readiness, safety systems, or compliance operations are realistically easier to move into from this type of experience. Any honest/practical advice would genuinely be appreciated.
