Multiple Reactions to one attack.

Hello I have a question about reactions. My friend (who is also new) said, that you can only have one reaction to an attack, only if the other player reacts to his own attack with an All-Out Attack, you can react twice. But I have found only this part in rule 5.2:

Use Reactions: Starting with the player using the ability, the players alternate using any abilities with an appropriate Reaction timing. Players can choose to pass instead of using a reaction, but once both players consecutively pass, no further reactions to that ability can be used.

There is stated that both players need to pass to stop the reaction process.

My question is:

On one attack, can I use All-Out-Defense, my relentless discipline Ward 5+ reaction and the Mortek Guard reaction ability that can support a friendly unit that is in combat range?

Can I block two damage points with Bolster Creation and Immaculate Defender?

Hi, does these two effect stack in the combat phase? Can I block two damage points?

Immaculate Defender (Heroic Trait)

Effect: Ignore the first damage point allocated to each friendly Ossiarch Bonereapers Infantry unit in the combat phase while they are wholly within 12" of this unit.

Bolster Creation (Spell)

Declare: Pick a friendly Ossiarch Bonereapers Wizard to cast this spell, pick a visible friendly Ossiarch Bonereapers unit wholly within 12" of them to be the target, then make a casting roll of 2D6.

Effect: Until the start of your next turn, ignore the first damage point allocated to the target in each phase.
