How am I doing?

I am a long time listener of Whitecoat Investor. I’ve listened to basically every podcast, especially Milestones to Millionaires. I made a promise to myself that I would follow the advice when I got out of residency.

Looking for an objective assessment on how I have done, what I can do better, and mistakes to avoid.

For context, I come from a low income family and I do support my parents so I finished residency with maybe $10k in savings and $0 in retirement and investments.

-I am 2 years out of training
-I paid off ~$200k in student loans
-I have around $600k between my retirement accounts and personal brokerage
-I am still renting so I do not own a home
-My goal is to hit $5 million in 10 years
-Household income is currently $900k (yes I have a very high savings rate)
-I want to buy a house because I have 2 kids but interests rates are high and houses in my city are around $2-3m because it is HCOL

I am proud of myself because of my high savings rate and a huge swing since I have been out from residency but I am working a ton and I worry that I will regret missing time from family. At the same time, I want to take advantage of my early earnings years while kids are very young to maximize my investments so that I can FIRE in my mid to late 40s.

Any advice? Thank you guys in advance!

Have I reached Coast Fire now?

32 years old, have ~$800k between my retirement accounts and taxable brokerage. This was all due to working a ton and having a very high savings rate and living within my means. I don’t have a fancy car or house but that’s not important to me.

I still plan to add monthly to my investments of course but am just curious if I can safely say that I’ve reached COAST Fire?

Genuine Question for my colleagues: how much does your Medicare procedure pay you?

I work in anesthesia but have always wondered. How much does Medicare pay you for your procedures?

An EGD or colonoscopy for example? Or a lap chole? Or a cysto/stent? What about a cardiac cath? Or a c-section? Etc

I have always been curious but very shy to ask my surgeon colleges. I hear that reimbursements have gone down a ton.

For example, my pain friends told me a SCS implant barely pays much anymore compared to before.

Is it a bad idea to buy a $2.5m house if my salary is $1.2m (pre-tax)?

I am in a tough situation. Need a large house for my family and my parents (I support my parents entirely and they don’t own a home). The only houses that allow for this much sqft are in the $2-3m range in my city? Household income is $1.2m pre-tax. No debt. We have around $600k in investments between 401k and private brokerage account. Around $100k in savings. My main concern is current interest rates of course. I know I can afford it but I don’t want to be house poor.

Edit: I have been out of training for 1 year. I am honestly leaning towards saving for 2-3 more years before buying such a home.

No budget. Prefer to stay in continental US, want a nice relaxing place with beautiful scenery. Plan to go in June/July
