How Much Should CEO's Make Compared to Their Employees?

>You do not earn a billion dollars. You steal it.

This is often implying that billionaires don't pay their employees enough. An employee can work for an hour and in that same hour, a CEO can make 1200x as much money.

Questions

How should pay be determined between CEOs and employees if the capitalist system isn't fair?
Should the CEO make the exact same amount of money as a low-level employee?
In that same hour, is the CEO providing more 'value' to the business than the low-level employee?
