Notice from EDD about potentially false claim regarding eligibility?

I just received this letter in the mail. The notice is about my eligibility being impacted and the notice of a potentially false statement.

The prep questions ask why I left my job, how that information may have been incorrect, my receipt of wages from my prior employer, and information provided above my earnings.

I have no idea if they mean earnings from the employer that let me go, or the employer before that.

I was only with the employer that let me go for 4 days and they claimed that we were incompatible and marked my separation as a layoff. Because of this I answered on my form that it was a layoff. This employer had also just transitioned 1 business day prior to be partially owned by a bigger company and HR. This means our previous pay stubs were linked to an old payroll, and we had just made new accounts for our new payroll that didn't transfer previous data. I dont know if this caused confusion because the termination was given with paperwork that did not mention the new partial owner. I do not know if this was communicated from the manager to the new hr department.

Is this type of notice usually due to the employer disputing, or could I have certified a week wrong? I just dont know the exact reason based on it just saying an eligibility concern. I dont have my phone interview for about a week. This is stressing me out a lot, and it would be helpful to know what triggered this notice.
