Anyone else have great erections when first starting TRT, then have them fade as testosterone levels increased?

Looking for others who have experienced something similar.

I’m 47 and switched from pellets to testosterone cypionate injections about 9 weeks ago. ( 200 mg weekly split in two injections)For the first 5–6 weeks, my erections were incredible. Morning wood every day, stronger erections with my wife, and everything seemed to be working better than it had in years.

Over the last few weeks, though, my erections have declined. Morning erections have mostly disappeared, erections aren’t as firm during sex or masturbation, and I sometimes need Trimix when I didn’t before.

My recent trough labs (drawn Monday morning before my injection after a Thursday morning injection) were:

Total Testosterone: 1,658 ng/dL
Free Testosterone: 201.7 pg/mL
SHBG: 52
Estradiol: 89 pg/mL
Hematocrit: 54.2%

My TRT doctor wants to keep my testosterone dose the same because my body composition and gym performance have improved significantly.

Instead, he increased my DIM dose and plans to recheck labs in 3 months. If estradiol is still high, he’ll consider adding an aromatase inhibitor.

Has anyone experienced something similar where erections were fantastic when starting TRT but declined once your testosterone and/or estradiol increased? If so, what ended up being the cause, and what fixed it?

I’m interested in hearing real experiences rather than just theory.

First time

Yesterday was my first-ever Trimix injection, and I wanted to share my experience in case it helps someone else who’s as nervous as I was.

Medication: Olympia Pharmacy T105
Starting dose: 5 units (0.05 mL)

For context, I’m on TRT and don’t have severe ED. I can usually have sex naturally, especially in the mornings, and Cialis helps. My biggest issue has been situational/performance anxiety during lifestyle events, plus I wanted to avoid Viagra because of ear side effects.

I spent weeks overthinking everything—the injection, the dose, whether I should start at 5 or 10 units, etc. Olympia recommended starting at 5 units and increasing by 5 units only if needed.

The injection itself was honestly much easier than I expected. I did it manually with an insulin syringe, and it was nowhere near as bad as I had built it up to be in my head.

The results:

Started working within about 10–15 minutes.
Had approximately 2 hours of amazing sex with my wife.
Stayed fully erect even after ejaculation for a while.
It gradually softened on its own over the next hour or so and was down to around 25% before I went to bed.
Mild fullness/aching during the erection, but no significant pain.

No bruising or problems the next day.

My biggest takeaway is that I’m really glad I listened to Olympia and started at 5 units. Before trying it, I was convinced I’d probably need 7–10 units. After this experience, I definitely won’t be increasing the dose. If anything, I may eventually experiment with a slightly lower dose for shorter sessions, since Olympia told me I could titrate up or down in small increments.

Overall, it completely exceeded my expectations and gave me a lot of confidence going forward.

For those of you with more experience: based on this response, would you stay at 5 units or consider trying 4 units for a shorter duration?
