How does productivity work in other SPDs?

I've been at my current hospital for about 7 months now, but recently they changed our productivity metrics. Due to that, we haven't been able to meet productivity in about 5 weeks. On top of that, they're starting to push us hard for personal productivity as well. Currently, it's measured by instruments sterilized, both for personal and the department productivity.

However, this doesn't include anything else. Even assembling trays doesn't count, only if they get sterilized. What's ended up happening is all of the techs are essentially beginning to fight each other. Nobody wants to go to decon, nobody wants to pull carts out of the washer, nobody wants to put trays up. They won't even grab small trays anymore. All they want to do is grab the large trays from the washer, or even pull 60+ instrument trays from the core itself, and nothing else. Then, they brag about how they've met their productivity 4 hours into their shift.

We're a small facility, 6 techs (one of which is part time), with an average case load of about 17 a day, primarily ortho/sports medicine. Frankly, it's really starting to fray my nerves because we used to actually work together. Now people are actively ignoring any obvious work that needs to get done if it isn't assembling trays, on top of actively shit talking the two techs (myself and one other) who end up having to be the ones to go to decon, to put trays up, to pull stuff out of the washer and the sterilizers, etc because they 'dont want their productivity to go to anyone else.'

Is this how productivity is measured in all SPDs? I've only worked in one other, and our manager never said anything about productivity in my old one.

Skin thickening, pain, hypervascularity, and BI-RADS 4 but no one seems to be taking it particularly seriously?

I feel like I'm at my wits end and don't know what to do. For going on 3 years now, my right breast has been swollen on the right side, starting past my nipple. It's painful, tender, and often I get stabbing pains through it. It's heavier than my left and it is much redder, but until about two months ago, no one gave it much thought. They just kept saying "Wear a better fitting bra as often as you can and lose weight."

I got another mammogram, my 5th one at this point (mind you, I'm not demanding they give me a mammogram. The first 4 were from two separate offices (my gyn and a breast specialist they sent me to), one yearly from each of them because I keep continuing to complain about the pain and swelling. The 5th one is because I moved states and started rebuilding my healthcare network), and it finally showed something to warrant an MRI. It was a patch of asymmetrical dense tissue that wasn't there on any other mammogram, but it was on the upper right side of my breast, not even where the swelling is. It was BI-RADS 0. Did the MRI, now it's BI-RADS 4 with an immediate biopsy recommendation. Granted, it's only showing that there's "diffuse asymmetric trabecular thickening, enhancement, and hypervascularity". There's no masses or anything like that at all.

So, I go to the surgeon to plan the skin punch and he seems confused that I'm even there. He tells me that he doubts the biopsy is going to come back with anything, so I ask what's the plan to deal with this if the biopsy is clean? And he hits me with the "I'd recommend you lose weight."

Does weight lead to asymmetrical breast skin thickening? I feel like I'm going crazy. The MRI even said that my right breast is the smaller one, but just looking at it shows the complete opposite. Do I just not know how normal breasts look? Does everyone just have asymmetrical breast pain near constantly?

Any custom tailor shops?

Like the title says, I'm looking to get a couple specific tops and an having a horrible time finding them online. Essentially, I've got one top that I got at Ross I want to get more of, but the brand doesn't appear to actually make clothes anymore. Are there any tailor/seamstress shops around that could custom make some tops?

[F4A] Fantasy, fantasy, and more fantasy!

Good morning! I'm Em, or Emerald, a 28 year old woman, and in EST. To get all the important bits out of the way before I get onto potential story ideas: Would prefer 23+ for any potential partners age, I use only discord to write, and I average anywhere from 2-6 paragraphs and would appreciate the same. While I can write anywhere up to novella, I've found over the years that it burns me out after about a month. Smaller posts allows me to write easily from my phone while I'm at work. Not to mention it's a bit more fluid in terms of dialogue.

I figured it would be best to narrow down my ideas and post a few ones out there, so today it's all fantasy!

The Druid and the Hunter: Thymae was a massive forest in the heart of Laevo, a sprawling kingdom that dominated the central continent. Yet, for as long as time remembers, no one has ever been able to cut into it, let alone go in and return unharmed. The wildlife are more ferocious and even the flora seems ready to trip and tangle anyone that dares to stride forth.

For centuries Laevo has been fine with leaving it that way, the superstitious kingdom at peace with the oddity, but now the new Queen has declared a need for greater resources. She wants Laevo to become a grand producer of timber for the rest of the continent, but that requires Thymae to be encroached upon. What few expeditions have been sent out to see which section of the forest would be best for cutting never returned. The peasants are getting nervous, afraid to follow the orders of the Queen, despite the glares of her soldiers.

The very same peasants cry out, recounting stories passed down through their generations. They claim that the forest is haunted by the spirit of a woman, stolen from her village by the forest and turned into its guardian.

That's where the hunter comes in. Hired to find whatever source of evil lurks within and either destroy it or neutralize it by any means necessary. (Your character doesn't have to be a hunter, either. They could be someone who got lost in the forest, or anything like that!)

A Goblin Paladin?!: Tanathil Elaren is not a name that evokes the appearance of a goblin upon first hearing it, but that's what the proud, smiling face of the goblin standing before you tells you. The woman is clad in gleaning plate armor, a shield on her back and a sword at her hip, ready and excited to be recruited into an adventure. When questioned about herself, she seems a bit confused why everyone asks. She is a paladin of the Ancient Order! She upholds timeless oaths and wields her power to protect all, even nature! Why does everyone only seem to talk about goblins when she's around?

Oddest bit of all, she entirely believes it. She's as devoted as any paladin ever would be, claiming that's how her parents raised her. Now, stop asking questions and let's get out to drive those bandits out of the neighboring city!

These, and a few others, are the genres I tend to stick close to, but they aren't hard and fast. Adventure and politics ends up coming through more often than not. A lot of them overlap, too.

These are currently my top fandoms that I'm interested in. I'll admit I'm no loremaster when it comes to any of them, but I have at least surface-level knowledge, if not more. That being said, I'm not the biggest fan of playing as or with canon characters. For me, fandoms are purely for RPing using the setting and having the named characters around. As an example, for Arcane, my OC is part of the Fireflies, so she knows Echo loosely, but doesn't hang out with him or know him well personally. I also do enjoy using fandom settings and either mixing them together or adding/taking away from them. Ultimately, it's a basis for a story that I enjoy customizing with my RP partner.

Admittedly, I have a plethora of ideas and OCs. These range from fandom ideas to original worlds to individual characters that are flexible enough to do anything. Essentially, I just want to write and so far I've gotten little more than ghosts when I've reached out to others. I prefer to craft worlds and ideas with my RP partner, turning it into a collaboration that will keep both of us engaged and interested.
