Postpartum disability extension- what do I say?

This may or may not be the right place for this, but I have an appointment with my doctor tomorrow. I'm supposed to go back to work next month and am NOT READY. I have to start pelvic floor therapy (she's the one who recommended/said its necessary) and I have had several back spasms that left me unable to walk without pain. I also had this horrid symptom called Dyphoric milk ejection reflux- which made me feel incredibly nauseous. I'm breastfeeding and reviving from pregnancy BV that keeps reoccurring since pregnancy. Ugh so many symptoms.

I also am starting therapy for pretty severe PPA... which leads me to I have anxiety about asking her to put me on disability. I know I have perfectly good reason but idk why doctors have this shame factor. Worse case is she says no. What's the best way to ask? Any advice helps.

She gave me an extension already during pregnancy so maybe that's why I feel shaky about it.

Not to mention I am really having a hard time letting anyone watch my baby. My in-laws love him but I don't see them as capable enough to care for him while I'm at work. My husband works full time.

Certifying late?

I am on disability for pregnancy and accidentally certified a few days late. It technically says you have 20 days to certify, but now my payments have been stalled for over a month and I haven't been able to contact them of course. It says my claim still active, and that I need a certification, but no forms are available when I go to inbox. Has this ever happened to anyone? What did you do?
The phone line is impossible it seems.

Edit: all while this was happening my doctor sent in an extension - that they recieved and documented recently, but that still hasn't lifted this issue.

Vaginal dryness hell?

Idk if anyone else has experienced this... but when I was pregnant I developed really itchy skin all over and especially around my vagina. I had every test done and everything came back negative. I had to take Benadryl everyday my third trimester. No cream or topicals worked long term. I thought for sure it would go away after I had the baby. Again, all tests negative... they say it's just hormones or "VAGINAL DRYNESS" -- my vagina is itchy nonstop all day it's unbearable and I can't take it anymore. Has anyone else had this issue during pregnancy and postpartum? I have done sitzbaths they do provide some relief. Have also tried all kinds of topicals the healthy kind and total chemical stuff. Nothing is helping. I try not to itch but sometimes that's all that relieves it. A vicious cycle truly.
