When you were in medical school and residency what was the appeal of clinic that drove you to it? How many extra hours are you working per week?

Lindsay Clancy Trial

Hace you been keeping up with how the defense attorney absolutely eviscerates the doctors notes? The guy is putting on a clinic like he has done this 1 million times! If I have learned anything from this, it is to keep my notes as short as possible, do not include any thought or opinion, only write down what the patient actually says, only include objective data, and to be leery of dictation apps. Also, if I put something down, I must act on it.

What are your thoughts?

What has been the most unique disease presentation that you have done a case study on?

Current PGY-1. I got 2 this week on nights.

Potts Disease (1-2% of TB patients worldwide develop Potts; patient denies history of pulmonary TB)

Serotonin Syndrome with temporal relation to increased dose of PCP-compounded GLP-1

Target Date Fund for Retirement?

I am a 32 year old male, in my 1st year of medical residency, earn ~$80k/year, and contribute 5% to my retirement (max matching). My retirement accounts through work are with Fidelity and I am 100% invested in BlackRock's 2065 target date fund. I set this up as a Roth.

Are target date funds wise for non-brokerage retirement accounts? Could I be doing something better?

How do you talk to patients about Palliative Care?

I am a PGY-1 and I had a patient with stage IV oropharyngeal squamous cell carcinoma that came into clinic today. My attending thought it would be good to talk to him about palliative care as they still wished to continue treatment.

Can you tell me how you bring up, describe, and talk to patients about what palliative care is?

Consolidate vs Not Consolidate

I have numerous federal student loans. I am working in a PSLF field and plan to do so for the next decade. My goal is to have my loans forgiven through PSLF. I plan on enrolling in RAP on July 1. My income for 2025 was $0 (I filed).

My question is this, given that I have numerous student loans, under RAP in order to pay the $50 minimum, will I have to consolidate my loans into one giant loan, and if so, will I lose out on any payment plan options?

Loan Repayment Programs While In Residency

I will be starting my FM residency program this July in a rural region of California. Is anyone aware of loan repayment/forgiveness programs that medical residents qualify for outside of PSLF? I plan on working in a Health Professional Shortage Area (HPSA) after residency. I was looking at NHSC programs but my understanding is that residents do not qualify, you have to either be in medical school or done with residency. Please correct me if I am wrong.

Level 3 Prep

How did you prep for Level 3 during residency? Which QBank did you use? Are Dirty Medicine (OMM/Ethics) and Randy Neil Biostats still useful?

I am starting FM residency this July. I planned on doing 10 questions/day on average and saving CDM cases/biostats/ethics for a month out. I plan on sitting for the exam early next spring.
