Inexperienced psychiatrist?

Can viladodone and seroquel work? They're saying it's for MDD and PTSD but I think they're missing the mark, he's high functioning-- so tricky. She's only been practicing for 3 years. I hope they can course correct bc it's really bad since he started the meds or not better at least -- and he's smoking weed too now. Hard to watch him having outbursts and getting in trouble at work and at home. Paranoid, grandiose, talking nonstop. Texting like crazy, sometimes inappropriate on family threads. Exhausting. He's been in DBT for 2 + years but is fixating on childhood stuff. Has gone NC with my parents, who are elderly. Getting into fights with strangers. Security being called on him at mall, hotel. Scared, sad but also resigned.

We know that as of the last six weeks or so he has been on Seroquel and vilazodone. We don't know the dosage. He said it's for major depression with PTSD, but I'm not sure his psychiatrist is experienced. She's only been in practice for three years and specializes in women's health. His DBT therapist also seems young. He is currently doing inner child healing, is low contact with my parents and has yelled at staff members at his building. My parents don't want to be the one to tell him he has to leave- they're old and exhausted- they want his therapist to break the news because it seems like he is feeling there is a conspiracy where everyone is against him and is also in trouble at work, my family is wondering if he needs to be hospitalized I don't think he will go voluntarily.

He basically said that he has been optimized by his Doctors with a cocktail to engage with his union at work and fight corruption -he seems to think they nailed it, but his episodes have not gotten better except for him going low contact with my parents due to childhood trauma which seems less manic. I'm wondering as a sibling what is my role?

What should I be doing? I have two small kids that need me. I have tried everything over the past couple decades, telling him to go to therapy and that I'm scared for his safety bc he gets into altercations-but my concerns have always been either minimized or ignored and now although he's in therapy I feel it all coming to head. He has been untreated and undiagnosed for so long that I wonder if he is giving a full picture to these professionals in the meantime, if he has to leave his apartment I'm not sure where he's going to be able to go. Any advice or support is appreciated. I am very new to the idea of having to hospitalize him, but it seems to make sense because he cannot control his angry outbursts and is about to lose his housing.
