Need advice regarding severe evening/night episodes, possible Concerta rebound, and worsening emotional state.

She is currently taking:

- Concerta 36 mg at 9 AM

- Cilapam at night

- Dayvigo 10 mg at night

- Zaptra at night

- Melzap only as SOS

Initially the episodes used to happen mostly at night while trying to sleep. Her mind would keep racing and she would suddenly feel like she was choking/unable to breathe. During these episodes she becomes extremely restless, agitated, and emotionally dysregulated for 1–2 hours. Sometimes she becomes aggressive, tries to bang her head against the wall, resists SOS medication, and is very difficult to calm down safely.

Earlier she was taking Concerta at 11 AM. We shifted it earlier to 9 AM, and now instead of nighttime the severe breakdowns are happening in the evening. This timing shift makes us wonder if this could be stimulant rebound/overstimulation from Concerta 36 mg.

Another major concern is that Concerta no longer seems to help as much as before. Initially it improved focus significantly, but now despite taking it she still feels extremely low-energy, emotionally exhausted, and says things like:

- “I’m done with life”

- “Surviving takes too much effort”

She mostly just eats and sleeps and has very low motivation/energy overall.

Today she also had a sudden ~30-minute episode while watching a movie where she developed:

- weakness

- numbness

- vision changes

- difficulty speaking

- heavy-headedness

- trouble breathing

She improved after taking 2 Melzap 0.5 and sleeping, but afterward still felt exhausted and heavy-headed.

Her tests were mostly normal:

- thyroid normal

- Vitamin D low

- B12 borderline low

- ferritin low-normal

We are already following up with her psychiatrist, but I wanted to ask if anyone has experienced:

- severe stimulant rebound like this?

- Concerta helping initially but later worsening anxiety/emotional crashes?

- emotional dysregulation/panic during stimulant wear-off?

- improvement after lowering Concerta dose or switching to lower Concerta + short-acting methylphenidate (Inspiral)?

- stimulants becoming less effective while anxiety/depression worsened?

Also curious whether this sounds more like:

- stimulant rebound/overstimulation

- severe anxiety/panic

- burnout/nervous system exhaustion

- depression

- or something else entirely.

Any experiences or insights would really help.
