Missed Achilles rupture

Hey all, educational question :)

I'm a last year resident and, in my last shift, I had the patient with a painful calf at the level of Achilles after he mistakenly stepped during the walk. Few years ago, he had an Achilles rupture on the other leg and, comparing both, he said this injury was far less serious for him compared to the previous. Clinically, he had no hematoma, no palpable gap, Thompson was negative (i squeezed his calf like 5 times to be sure). He was barely limping. We do not usually spam ultrasounds for every suspicious tendon rupture at the hospital but I usually do them on my own just for the sake of educating myself. This time, having a busier ER than usual, I decided to skip it as it seemed to me unlikely that he had a rupture based on his history and physical exam.

Today, I found out he went to another hospital 12 hours lated and was diagnosed with the ruptured Achilles and positive Thompson. Now, I do not know if he had another injury in the meantime but I do not think so. Have you ever experienced that? Do you proceed with the imaging regardless of history and physical exam in these situations or you rely on clinical exam more? I know other calf muscles can compensate for the ruptured Achilles and have observed some of those but they were still looking far worse than this patient was.

EDIT : I wrote he had a surgery first but it was misinformation from my department. I called my colleague who’s working at that hospital and it was a high rupture that will be treated conservatively. Still, I missed it.

Surgical preparation

Ladies, gents

How do you prepare for surgeries as an attending?

I’m soon to be done with mine residency (non-US), and looking for a nice online sources.

I’m currently mostly using AO Surgery ref and youtube when doing my cases but would really appreciate any other recommendations , especially anatomy related? Heard bout KenHub but haven’t used it, is it good?

E-reader for academic papers that sync well with iOS

Hey all,

I own a Kindle and just entering the world of e-readers. I tried reading my academic papers on old Kindle but it’s not working as I expected.

My life will soon get bit more busier so I’m searching for something I can read papers and books on before sleep since that would be my only dedicated time to read :).

I’m looking for something that sync well with apple exosystem, I’m using Zotero for my academic papers on my Mac.

I currently own an iPad but I guess that’s out of table for night reading?

Podcasts on the go

Hey peeps,

I’m soon to be done with my residency.

Now I’m in a search for the podcasts to keep me updated on the literature once I’m done and to keep me fresh on the ongoing research

20-40 minute episodes would be ideal to listen on the go or when I’m strolling my kid to get the best use of the time when he’s sleeping :)

If it means anything, I’m interested in lower limb sports injuries and hip/knee replacements

Any recommendations? Free or paid, it doesn’t matter, I’m willing to paid if something is good.

Keychron M3/ M3 mini

Hey peops, Mac user here. Until now, I was using only Logitech mouses and wanted to try something new. I opted for the Keychron M3 regular since the local price was good and it was quickly available.

I was using it for few days now and the first impression is great. With the Mac Mouse Fix it works like charm, almost like a product made by Apple. Having said that, I immediately started thinking about buying 1 more - M3 mini to be my portable, out-of-the-house option?

Now, since I've read some bad bad things about their customer support, I am a bit sceptical now with buying another one immediately and decided to ask you here a bit about some of your previous experience with Keychron and their products and would any other mouse be a good portable option instead of mini? Keychron or any other? What is the longevity of Keychron mouses?

Keychron M3 longevity

Hey all,

I just got my Keychron M3 mouse as a Mac user and I have to say the mouse feels amazing. With the Mac Mouse fix it basically feels native to the Mac both for tracking and scrolling (though less but still pretty good :) ).

I've read horror stories here about their customer support but heck, the local price was pretty good.

What is your general experience with their mice regarding the longevity, how long could I expect it to last? I have thought about ordering another one - probably M3 mini version for the portability. Would you recommend any other instead?

Air + Studio

Hey peeps,

I’ve been a Mac user for few years and the experience has been amazing. I currently own Mac Air, ipad Air and Iphone ofc, and now I am moving to a bigger apartment so I was thinking of buying mac studio and selling iPad as I am not really using it anymore after I finished my studies but I need something portable for travels such as Air , hence no Pro needed for me

How’s the experience owning both? Any advice regarding the Studio? I’ve only ever owned dekstop PCs and have no clue about bigg ass Mac machines.
