Can someone explain to me how would these adapters work/is it safe? Cuz the logic hasn't clicked on me yet.

I know I've stumbled upon these kind of stuff they sell. I've never owned one cuz I don't have the appropriate PSU. But I wonder if someone could make a YT video how these work safely? Cuz it's doesn't makes sense to me nobody is talking about this in the West
