Do I need to add a form to the website? I don't want to do that. I just want the website to look like:

So yep the problem is an event at the zoo. Which requires more walking than is reasonable for this guest.
My question is, do I drop the guest off at the gates and then rent a wheelchair, at the Zoo gates, or do I rent a wheelchair from off-site for the day?
I don't know if I will be parking at the North or South side. Until we sign up for this event.
The guest uses a cane around the house, but, in my opinion and their opinion, cannot walk 10 minutes in or out of the Zoo.
If you've heard him read the news late at night, you'll know who I'm talking about. Just a very strange whispery voice, strange cadence; doesn't seem to fit the style of a CBC newsreader at all. Unnerving.
