Help for 4 month old

Please excuse my English, it’s not my native language, but unfortunately there isn’t a suitable subreddit in my language. So i‘m here hoping for any help or advise. Oh, and sorry for any mistakes for the a.m and p.m thing, in my language, we don‘t have that 🙈.
I need help improving my baby’s sleep. My baby is now 4 months old. Since birth, she has never slept well.. at most 2 hours at a time during the night.
We’ve now gone through the 4-month sleep regression, and I was somehow hoping things would improve afterwards. But that hasn’t happened. Do you have any tips on how I could structure the day better so she can sleep more at night?
Our daily routine looks like this:
Wake up around 8:30 a.m.
Awake for about 1 hour 45 minutes to 2 hours
Then sleeps for 30–40 minutes
Awake for 1 hour 45 minutes to 2 hours
Then sleeps for 30–40 minutes
Awake for 1 hour 45 minutes to 2 hours
Then sleeps for 30–40 minutes
Then awake for 2 hours before falling asleep for the night.
We co-sleep in my bed. At night, she wants to breastfeed a lot, although she also drinks well during the day. Often, she is awake in the middle of the night around 3 or 4 a.m. for 1–2 hours, and getting her back to sleep is hell.
She is very attached to me, can‘t stand to be left on her play matt without me interacting with her. I can‘t even sit right in front of her, she needs me to really play. The first 12 weeks, she would only cry and scream for hours.. we went to her doctor, that told us she is fine but a high need baby.
She does her naps in a motorized baby hammock (hope thats the word).
Do you have any tips on how I could structure the day better to have a better nightssleep?
