The hull went out the barrel and downrange?

I was patterning a 1301 and found that one of the hulls was shot out the barrel and embedded itself in the target 20 yards away rather than ejecting like normal. I've never seen or heard of this happening and am unsure if it's an ammo issue, a mechanical issue, or something else. Does anyone have an idea of what could have caused this?

Is this normal?

I got back from the range and noticed this line straight across on the interior of the crane of 642 uc. I don't see a similar line on my other J frames and cant tell if this is a cosmetic issue resulting from the slightly different lug or a genuine issue.

It goess straight across in a line and isn't visible on the edges, but running a finger across it, you can feel a crack or separation is present. I.e. it isn't a smoothe line of carbon.
