Splitting 5mg dex tablets
TLDR: Dr says tablets are made to be split for easy swallowing, and that a 2.5mg dose is fine, but that I can’t split a 5mg tablet and take a 2.5mg dose.
(Just to preface, I will be listening to my prescribers advice, but am just curious if anyone has an explanation)
Hello all, I recently had a conversation with my prescriber and it’s left me a bit confused. For the last 4 weeks I’ve been prescribed 60mg of elvanse and a 5mg dex top up. I had tried 70mg but found it a bit too intense when it kicked in, and also found it wore off sooner than I’d like so we agreed to drop the dose and then take a top up later.
I found that the 5mg was messing with my sleep in the evening + also that the elvanse was taking too long to kick in in the morning, meaning I still found myself rushing around to leave for work if I was doing an opening shift (time blindness is a big issue for me). So I played around with it a bit and started splitting the 5mg dose, so I would take half a dex tablet first thing if I was working in the morning + then delay my elvanse until about 9am. And if I was doing the closing shift (and therefore didn’t need to be raring to go first thing in the morning) I would take my elvanse in the morning + then half a dex tablet at about 1pm which sustained my focus long enough for the rest of the day. (Total dose of 60mg elvanse and 2.5mg dex on any given day).
This has been working great for me, has removed any sleep issues and has increased my productivity in the mornings, but when I told my prescriber this today they said I couldn’t do it as the dex tablets aren’t made to be a split dose. I said that hadn’t occurred to me as the tablets are instant release, non-coated, and have the scored lines in them to be able to split the tablets and so I hadn’t even considered that it would be a problem (I understand medication delivery mechanisms and wouldn’t be messing with extended release or capsules or anything). They said the lines are there to split the tablet for ease of swallowing and that they could look to prescribe me a 2.5mg dose instead.
This is where I’m confused- if the tablet is made to be split for ease of taking, then splitting it isn’t the problem. And if they can prescribe 2.5mg, then the dose isn’t the problem. So where is the issue with what I’ve been doing? I can only assume it’s them covering their backs with following official guidelines, and maybe the risk of uneven dosage across the two halves of the tablet, but if I’m allowed to take the FULL tablet then surely that wouldn’t matter? I will be following the prescribers advice going forward and will just play around with the time I take my medication but it just seemed a bit nonsensical to me.
