Demotivated after 3rd ER. DOR and tubal factor. Will it ever happen for me? Does changing protocol rly help?

Hi all, as the title says I am quite demotivated after 3rd ER. - Got 2 eggs but 1 degenerated so left with 1 mature egg.

I am 34 with DOR/very low AMH (~0.2), recurrent hydrosalpinx after prior salpingotomy. Never seen a double line ever.
First 2 fresh cycles (untested) failed.
Always had issues with lead follicles and asynchrony

Prev cycles:
Cycle 1: Antagonist cycle with norethisterone priming 1 week
Pergoveris stimulation
Dual trigger
~4 follicles initially seen but only 1 MII egg retrieved (others appeared empty)
1 Day 2 embryo transferred fresh → failed

Cycle 2: Long agonist / prolonged suppression cycle
Long suppression with triptorelin ~2 weeks before stims
Pergoveris stimulation then single trigger
2 mature eggs retrieved, 1 fertilised.
1 Day 3 embryo transferred fresh → failed

Current cycle:
Started genotropin (similar to omnitrope)
Stims with Menopur 150 + Rekovelle 12
Dual trigger

Retrieved 2 eggs:
• 1 degenerated
• 1 MII

Post retrieval scan still showed ~4–5 small follicles, so RE recommending DuoStim starting 11 May

I did try to ask briefly today about MDLF - my RE says there are many protocols but he doesn’t feel it would be much different. He is still thinking abt antagonist protocol next round but we can talk more after duostim retrieval.

TLDR:
34yo nvr pregnant with very low AMH AFC 3-4, recurrent left hydrosalpinx, with asynchrony during my first 3 IVF (antagonist cycle -> long agonist cycle -> antagonist)
(only 1-2 eggs each time). Now RE suggesting DuoStim.

Questions:

  1. ⁠Does this sound more like true DOR/limited recruitable cohort rather than “wrong protocol”? What worked for u and what should I ask for?
  2. ⁠Any success stories with DuoStim in this kind of pattern?

Feeling demoralised but a part of me still wanting to keep trying.. but I wonder if this is a losing battle…

    - For people with repeated 1–2 egg retrievals, did changing protocol (MDLF, estrogen priming, minimal stim etc) significantly improve yield?
  2. Any success stories with DuoStim in this kind of pattern?
  3. Would you continue trying with own eggs in this scenario at age 34?

Feeling demoralised but a part of me still wanting to keep trying

Hi all! I am going for my 3rd retrieval - I 2 leads that we are hoping to collect. My dr mentioned about duostim which I haven’t done before : basically after collection I am to rest for a few days then start stim again and collect

Is this considered luteal phase stimulation? It’s completely new to me. Anyone had any luck with this?

Also what should I ask my RE next cycle to improve the synchrony? What worked for u?

Just turned 34 2 days ago and now on day 2 of my stims. This will be my 3rd retrieval. 1st and 2nd retrievals I got 1 embryo (day 2 and day 3) each time. Failed fresh transfers. Then found out my swollen tubes came back again.

This time I changed clinic and dr, the protocol is new too - menopur and rekovelle (previously I was on pergovaris), with Genotropin (i think it’s equivalent to omnitrope) support and started DHEA.
We are planning to do only retrievals and he’s gonna try to make blasts rather than day 2-3 embryos. Then before transfers, I am going to have my diseased tube/s removed.
I AM SO ANXIOUS.

Please give me your happy stories and support.
Also , can I ask during this time

  1. What kind of exercises can i do? I know my risk for OHSS is rly low. But I am thinking is it still ok to run on treadmill?
  2. What are ur thoughts on ERA testing? Ivo recurrent transfer failures?
