incapacitated person form

father want's declaration of incapacity for his adult child. form is adult guardianship. the patient has autism but functional. i met the patient once... will probably have them defer to psychiatrist. wanted to know what hive opinion is on these kind of legal documents

new healthy medicare patient

without any past medical history, no meds, no chronic or acute concerns, establish care or AWV visit but as a new patient. never seen a doctor but few years ago insurance company sent somebody to his house to get labs drawn, reportedly all normal although i dont know what they checked.

what codes to get to get basic labs covered: cbc, cmp, a1c, lipid panel?

i know about signing abn. i would like to know if anybody else has any diagnostic codes that would help coverage though.

I need a elevated fasting glucose to check a hemoglobin A1c, but then I need a reason to get a BMP. It's very circular reasoning. Lipid panel you can check every 5 years but then how do you calculate ASCVD on a 3 year old lipid panel.

These are common, basic labs. They are not expensive. They are covered by every other insurance under an annual physical. It is unnecessarily burdensome coding. What percentage of your 65+ population has a normal lipid panel or a fasting glucose? I would argue it's near the minority if it isn't the minority already.

E&M coding distribution report

Received a letter stating my coding distribution is an outlier compared to the average billing distribution. Their expected billing distribution for 99214 is 51% and I'm 70%.

What do most people do with these? Cintas? I'm confident in my coding and to withstand an audit but that being said I don't want to go through that headache either.

I thought it about more and I think this is skewed. I am generally more relaxed and patients with chronic conditions that are stable I only have them follow up once a year in their AWV vs. having them schedule a 6month follow up, that is effectively the same thing billing-wise. I could technically have them follow up q4 or q3 monthly and do 99213 visits or try to be more efficient and clump it into q12 or q6 visits. Another reason to think about going into DPC or concierge..
