27 Male – Possible kidney issue (proteinuria + blood in urine) – what should I expect next?

Hi everyone,

I’m a 27-year-old male and I recently got some lab results that are worrying me a bit. I’m trying to understand what might be going on and what the typical next steps are before I see a nephrologist.

Here are my results:

  • Creatinine: 108 µmol/L
  • Urea: 11.2 mmol/L
  • eGFR: ~95 (calculated)
  • Urinalysis:
    • Protein: +2
    • Blood: +1
    • Dysmorphic RBCs: 8 / HPF
    • RBC: 13 / HPF
  • Albumin/Creatinine ratio (UACR): 74 mg/mmol

Other info:

  • I didn’t drink water for ~12 hours before the test
  • I do regular training (gym + running)
  • No known diagnosis yet
  • Blood pressure is normal

I’m mainly trying to understand:

  1. Does this pattern usually suggest a glomerular kidney issue?
  2. How serious does this look based on your experience?
  3. Is it possible this could improve/stabilize, or is it usually progressive?
  4. What are the typical next steps (biopsy, medication, monitoring)?
  5. Should I be worried about long-term kidney failure or dialysis from something like this?

I already plan to see a nephrologist tomorrow, but I’d like to understand what people with experience in this have seen before.

Any input is appreciated.
