Revision rhinoplasty question

Hi all I’m seeking a revision rhinoplasty because I did not achieve the results I asked for my primary rhinoplasty. I’ve attached my current nose now and an edit of what I want to achieve with the revision. Before anyone comments that it doesn’t look bad, yes I agree but the insecurity I wanted to correct is still there and it’s very frustrating. My tip is still pretty bulbous 2 years post- op and I feel like my doctor made my the bridge to tip transition a little off, as in there is a supratip break that i did not want and am hoping it can be corrected with a revision , I will ask for a straight bridge to tip transition. My question is what this type of surgical technique it would likely include? would i need any grafts or rib cartilage or would it likely just be through using sutures ? if cartilage or grafts are necessary, would they look bulky or visible and thick? for reference i have thick skin. Revision Rhino expectation??

Is it realistic to achieve a result like this? I’m getting a revision 2 years PO because I am unhappy with my round bulbous tip. Don’t know if I’m being delusional but I feel like the reference I chose have a similar nose shape except my alar base (tip) is wider than hers. But I can’t tell 100% and would like the opinion of others to know if this is achievable and realistic for a revision or not?

Rhinoplasty Accutane 1st pic me 2nd pic my reference

I’m 2 years post op and still feel like the base of my nose (my tip/nostril area) is wider than I wanted. I reached out to my surgeon about it and said I was considering a revision because of it. He said that he would like for me to begin on Accutane because he believes my skin thickness/oiliness is what’s causing the width and that accutane will control the oiliness and slim it down. He said if that does not help, we can re-evaluate for a revision.
I’m not sure if he’s only saying this because he can tell i’m not happy with his results or if this is actually helpful does anyone have any experience? If you’ve done accurate post-rhino has it helped and are the results permanent or is it just stalling and false hope?
