Help! Tiny pustules in the same exact spots

I keep getting these tiny little pustules/whiteheads on my cheeks and chin, and they keep coming back in the exact same spots. I basically never break out on my forehead.

I feel like my skin barrier may have gotten compromised after using tretinoin, so I stopped all actives and have been using only Prequel Glycerin Wash + Clinique Moisture Surge to keep things simple.

I’m also on spironolactone + doxycycline 50mg from my dermatologist, but I haven’t seen much improvement.

Could this be a compromised skin barrier, or does this look like something else? I’m scared to add more actives and make things worse. I’m also struggling to find gentle, non-comedogenic products that won’t clog my pores.
Would really appreciate any advice or product recommendations!

Trying to figure out what’s breaking me out 😭

Im 20 and used to have completely clear skin, but recently my skin has started randomly breaking out like this. It’s mostly tiny little whiteheads all over my face, and I can’t figure out what’s causing it.

I used tretinoin for several months and didn’t see much improvement, so my derm put me on 50mg spironolactone + doxycycline. I’ve been on those and it still doesn’t seem like my skin is getting better.

I’m supposed to start 0.03% adapalene soon, but I’m wondering if anyone has experienced breakouts like this or figured out what was causing them. Any ideas on what could be triggering these tiny whiteheads? I’m wondering if it’s a reaction to something
